Problem
A message window reads, Unable to load/create dialog item of type iconCmdFrame, id = 5680 from MGDSHOOK.
A second message window reads Unable to load/create dialog item of type toolbox, id = 548 from USTN.

Solution
MicroStation is trying to load a tool box (with an ID of 5680) and it canot find it. The easiest way to resolve this is to close MicroStation, delete the *.M01 file, and then restart MicroStation.
